Choosing a care home (checklist)

A checklist for people with MS, family and carers

This checklist can help you ask questions about care homes. So you feel more confident you’re choosing the most suitable home. You can read more about residential care on our web pages.

You can print our checklist and fill it in with a pen. Or save it to your computer or phone and fill it in there. 

Use the checklist on an Android phone 

1. One of the easiest ways to fill in the Care home checklist on an Android phone is using the free Adobe app. You can find it on the Google Play Store on your phone. Install the free Adobe app to your phone
2. Open the Care home checklist pdf 
3. Save the file so you’ve got a blank version for when you need it again.  Then save the version you’ll fill in. Choose ‘Save as’ and add the name of the care home. 
4. Fill it in as you go, and don’t forget to save the completed form. It usually saves automatically. 

Use the checklist on an iPhone 

1. Download the Care home checklist pdf to the Files on your iPhone
2. Select the file and then Save as (for example save as the name of the care home). That way you can keep blank version for when you need it again
3. Click the pencil icon with three dots, located in the bottom left of the screen. Then select each box on the checklist in turn to fill them in
4. It should save automatically
