Our Health and Wellbeing grants are there to help you pay for items that will have a significant positive impact on your life.

Changes to Health and Wellbeing Grants - January 2020

We've made some changes to our grants which affect what you can apply for, how much we can award and who's eligible to apply.

Please make sure you read the revised guidance notes before completing the application form.

All changes apply to applications received on or after 1 January 2020.

How to apply

To apply for a Health and Wellbeing Grant you need to:

  • Fill out an application form
  • Send it to us with a letter of support from a health or social care professional and two quotes for the item you're asking for.

    We'd also like you to complete an equality monitoring form, but this isn't part of your application.
