How to obtain a quote

before returning it to us by email at mssociety@heathlambert.com or by fax on 01603 625230. From your answers to the questions on this form, we will be able to assess your individual needs and requirements in order to provide you with an indication of a possible life assurance quotation.
If we are successful in obtaining an indicative quote and you would then like to proceed with your application, you will be asked to complete a full proposal form, which will be provided to you, in order that we can issue a tailor made quotation. Once you have completed this form, you can either email it back to us at mssociety@heathlambert.com or fax it back to 01603 625230.
